If your clinical team has shared a Pathway with you, they believe you are a good candidate for an independently managed digital care program, and that you can get back to your normal routine without traveling back and forth to the clinic.
In this article, we'll walk you through the account setup process so you can start managing your care, as well as how you can set up shortcuts for easy access.
Setting Up Your Account
To get started, tap the link your provider shared in your email or text message, or that you received by scanning the QR code.
To create an account:
- Enter an email address and password, then select Continue. Alternatively, you can log in with your existing Apple, Google, or MyChart account.
- Enter your full name and date of birth.
Note: be sure that your name and date of birth here match what your provider's office has on file. - To continue, you'll need to select I verify this information is correct.
-
Finally, review the communication agreement to determine whether you consent to have Medbridge communicate with you about your program.
If you select to consent, you can receive text or email reminders about your program including alerts if your physician messages you and the ability to report exercise activity without having to log fully into the platform. This can be helpful for keeping you on track with your recovery. If you do not consent, you will not receive these notifications.
If you agree to our Terms & Use & Privacy Policy, select the I agree to the Terms & Use & Privacy Policy box and Finish account registration to continue.
If your provider has already started an account for you, simply verify your name and date of birth to finish creating your account and log in.
Note: The information you enter here must match the information that your provider entered into the record, otherwise an error message may occur.
After you've logged in, the first thing you should do is look at your Tasks for Today and complete your baseline assessment. This lets you see how you improve as you progress through your Pathway.
Once your assessment is complete, you can review your assigned exercises and education, log activity, and more!
